Minimum Service Set Requirements:
Collection of Vital Signs
Ability to collect heart rate, respiration rate, temperature, and blood pressure in the pharmacy for patients.
Comprehensive Medication Reviews and In-Depth Counseling
Conduct thorough reviews of patient medications, including prescription, over-the-counter, herbal medications, and dietary supplements to identify, resolve, and prevent medication-related problems.
Targeted Medication Therapy Management (MTM)
Provide personalized MTM services based on patient request, referral from the Primary Care Provider, or pharmacist determination of high-risk patients. These services must be schedulable and conducted in a private setting.
Home Delivery
Offer pharmacy-provided delivery services, with charges and restrictions as applicable.
Immunization Services
Actively screen patients, educate, and provide ACIP-recommended immunizations, including influenza, pneumococcal, Tdap, and herpes zoster immunizations.
Produce Personal Medication Records
Create comprehensive lists of current patient medications manually or from dispensing software.
Care Plan Development/Reinforcement for Chronic Patients
Develop and maintain care plans detailing patient information pertinent to achieving healthcare goals.
Adherence Programs
Participate in one or more adherence programs approved by CPESN®, such as Adherence Reminder Calls, Med Planners, Strip Packaging, Blister Packaging, or special adherence packaging to promote medication adherence.
Practice Presumptive Eligible Medication Dispensing
Dispense medication based on the belief that the patient is eligible for Medicaid and is in the application process, to be billed to Medicaid once actual eligibility is obtained.
Quality and Data
Maintain comprehensive and accurate clinical records to support quality care and compliance with regulations. Participate in quality improvement initiatives and data reporting as required by CPESN®.
